Written Narrative
If you choose to provide a written narrative:
Keep samples of your child's work in different subjects, showing progress.
Pictures and other mementos are helpful.
Records of grades, activities, classes, etc. are usually needed.
You must find a certified teacher (a list is available to CHESCA members) to examine portfolio of your child's work and to sign an Academic Assessment form like the sample below.
Note: An academic assessment can be done by someone other than a certified teacher with prior agreement from the superintendent. For example, families have asked a friend with educational experience and obtained agreement with the superintendent that this person could provide the academic assessment.
A portfolio assessment (written narrative) by a pre-arranged agreement between parent and superintendent is protected by current Ohio state regulations. For more information call Earl or Diane Rodd at 330-305-9318.
Very Important!
Note: Send a completed Academic Assessment form with your notification form when notifying your Superintendent for the next school year. Do NOT send your portfolio with notification! Here is a sample form:
